Case note audit of missed SGA: Standardised clinical outcome review and evaluation (SCORE)

The GAP SCORE electronic audit tool reviews:

  • Risk assessment at booking and throughout pregnancy
  • Whether guidelines for serial scanning were followed
  • Were customised growth charts used appropriately
  • Referral criteria and process
  • Growth scan protocol
  • Summary section for comments and actions.

It is recommended to input 30 randomly selected cases in each 6-month period.

Cases should be deliveries of SGA babies which were not recognised antenatally. 
GAP-SCORE can be accessed following a brief training session by the Perinatal Institute 
(contact GAP@perinatal.org.uk )
