Case note audit of missed SGA: Standardised clinical outcome review and evaluation (SCORE)
The GAP SCORE electronic audit tool reviews:
- Risk assessment at booking and throughout pregnancy
- Whether guidelines for serial scanning were followed
- Were customised growth charts used appropriately
- Referral criteria and process
- Growth scan protocol
- Summary section for comments and actions.
It is recommended to input 30 randomly selected cases in each 6-month period.
Cases should be deliveries of SGA babies which were not recognised antenatally.
GAP-SCORE can be accessed following a brief training session by the Perinatal Institute
